Erectile dysfunction (ED) is a medical condition characterised by the inability to achieve or maintain a penile erection sufficient for satisfactory sexual performance.1 It typically arises from the direct or indirect dysfunction of an enzyme called phosphodiesterase type 5 (PDE5). It is responsible for breaking down cyclic guanosine monophosphate (cGMP), a molecule involved in the degradation of cGMP. cGMP is essential for the normal maintenance of an erection.3 When an erection persists despite sexual stimulation, PDE5 is blocked.4 In fact, PDE5 is the primary enzyme responsible for the degradation of cGMP,4 and it is postulated that cGMP degradation can be slowed by the inhibition of phosphodiesterase type 5 (PDE5).5
It is worth noting that ED can be caused, at least in part, by various factors, including cardiovascular disease, diabetes, obesity, high blood pressure, psychological or relationship stress, and certain drugs such as alpha-blockers.5
To date, there have been no trials of PDE5 as an treatment for ED.2 It is presumed to be effective only when it is used with caution in patients with diabetes mellitus (e.g. after radical prostatectomy or chemo-surgery).5,6
The Erectile Dysfunction (ED) Clinical Subcommittee (ES) of the Committee on Drug Interactions (CDI) has adopted a special report on PDE5 as an effective treatment for ED.2 The report highlights the challenges of managing ED in patients with diabetes mellitus (DM) and provides guidance on the appropriate use of PDE5 inhibitors in this group.2 It also highlights the importance of careful monitoring of PDE5 levels in patients with DM.
To date, no PDE5 inhibitor has been approved for the treatment of ED.2 ED is characterised by the inability to achieve or maintain an erection sufficient for satisfactory sexual performance.5 ED can be caused by various factors, including cardiovascular disease, diabetes, obesity, high blood pressure, psychological or relationship stress, and certain drugs such as alpha-blockers.
1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. It is contraindicated to use Cialis with nitrates or recreational "poppers" (amyl nitrites).
2.Alpha-blockers (e.g., prazosin, terazosin, tamsulosin): Both Cialis and alpha-blockers relax blood vessels, which can lead to excessive blood pressure reduction when used together. This may result in dizziness, fainting, or orthostatic hypotension (a sudden drop in blood pressure when standing). If needed, your doctor may adjust the dosage or recommend spacing the medications.
3.Other antihypertensive drugs (e.g., atenolol, propranolol, enalapril, lisinopril): Cialis can enhance the blood-pressure-lowering effects of these medications. While this interaction is usually less severe than with nitrates or alpha-blockers, it can still cause dizziness or fainting in sensitive individuals. Close monitoring of blood pressure is recommended.
Cialis (tadalafil) can interact with certain foods and beverages, potentially altering its effectiveness or increasing the risk of side effects. Here are the key drug-food interactions to be aware of:
If you are taking Cialis 50 mg once daily, you may experience side effects like these:

Nitrates, known for their blood pressure-lowering effects, have recently become a game-changer. They testified before the FDA and are recommended by healthcare professionals for treating angina, high blood pressure, and high cholesterol. They are commonly prescribed to individuals with ischemic heart disease, angina, and high blood pressure. However, their blood pressure-lowering effects can be uncomfortable, ducers of enzymes that break down nitrates, and they may interact with certain foods and beverages.
